Eligibility
Current Weld RE4 students at least 15 years old attending Windsor High School, Severance High School, or the WEST program who have a 504 Plan or an IEP (Individual Education Program).
Students attending college or training programs who had a 504 or IEP while in high school.
Youth up to age 24 who are no longer in school but had a 504 plan or an Individualized Education Program (IEP) while they were in school.
Students or youth who suspect they may have a disability, but not yet diagnosed.
Students and youth who meet the above criteria and are ready to begin career exploration.

Services
What are SWAP Services for STUDENTS?
Eligible students receive Pre-Employment Transition Services focusing on PREPARING for the world of work. Activities may include:
Career exploration
Work site tours
Career focused guest speakers
Volunteer work
Job trials
Interest and aptitude assessments
College visits and resources
Woven in to all activities will be an emphasis on developing essential workplace skills such as the ability to communicate, social skills, timeliness, commitment to work, willingness to learn, accountability, etc.
What are SWAP Services for OUT OF SCHOOL YOUTH?
Eligible out of school youth receive services that focus on ATTAINING employment. These services can include:
Exploring career interests
Work site tours
Work preferences such as location, hours, and trends in a specific line of work
Support completing applications, writing a resume, preparing a cover letter, and preparing for an interview
Job coaching may be available on the job site
Help find, apply for, and be successful at work
